The Gruesome Eminence is a post-Moon Lord magic weapon. It is the exhumed form of the Ghastly Visage. It fires a spirit monster that poorly follows the cursor and occasionally moves around randomly on its own. As time passes, it spawns small spirits which deal 140 / 280 to player and Town NPCs. After 14 seconds, it grows to full size and becomes fully controllable by the player, but also stop releasing spirits. The damage of the spirit gradually builds up from 1x to 3.3x as it builds up to full size, but will also deal 184 / 368 damage to the player until fully grown.
Its best modifier is Mythical.

Trivia
- The amalgam mentioned in the item's description refers to Polterghast.
- The interior of the spirit monster contains two different silhouettes resembling Giygas from the EarthBound franchise, and another resembling a fetus.
- The weapon itself bears a resemblance to the trinkets dropped by The Collector from Darkest Dungeon, particularly Dismas' Head.
